Overview

Pancreatic adenocarcinoma is a malignant epithelial tumour arising most often from the ductal cells of the exocrine Pancreas, and it is among the most lethal solid cancers because of late presentation, early metastasis, and resistance to therapy. The majority are ductal adenocarcinomas, frequently located in the pancreatic head, where tumour growth can obstruct the bile duct and produce painless jaundice, weight loss, abdominal or back pain, and new-onset diabetes. Because early disease is typically asymptomatic, many patients present at an advanced stage, occasionally with dramatic complications such as gastrointestinal haemorrhage from local invasion. Diagnosis combines cross-sectional imaging, endoscopic evaluation, and tissue confirmation, supported by serum markers; carbohydrate antigen 19-9 is the most widely used, although carcinoembryonic antigen and bilirubin may add predictive value at initial presentation, particularly outside specialist centres. Staging distinguishes resectable, borderline resectable, locally advanced, and metastatic disease, which determines whether surgical resection, the only potentially curative option, is feasible. Treatment integrates surgery with chemotherapy and, in selected cases, radiotherapy, while most patients receive systemic therapy aimed at disease control and symptom relief. Research focuses on earlier detection, better biomarkers, and more effective systemic regimens to improve the historically poor survival associated with this cancer.
